Thanks for visiting the Alpine Sports Medicine website. If you are on this page, chances are you've never visited our practice and are uncertain as to whether Physical Therapy is for you or not. We've compiled a list of frequently asked questions below, these are quite general but should give you an idea of what to expect on your first visit. If you find after reading this information you still have questions, please do not hesitate to call our offices (435) 645-9095.

Q What do I need to bring with me for my 1st visit?
A Just bring your doctor's prescription if given, our Patient Information Form (available
from the link above) and your insurance card. Some insurance companies do require
a prescription so be sure to let us know when you schedule the name of your
    insurance company.

Q What do I wear?
A Please wear something comfortable. We also have gowns to change into if necessary.

Q What do I do when I get there?
A Just take the steps or elevator up to our 2nd floor clinic, check in at the front desk and
you will have to fill out some brief paperwork. Then you are ready to see one of our
physical therapists for an evaluation of your injury to design a treatment plan to help you
    get back in the game.

Q How long does my appointment last?
A It depends on the nature of your injury, but generally between 1-2 hours. We like to
evaluate and treat your injury on the initial visit, so your initial visit might be a little longer.

Q Do I need to schedule with a particular therapist?
A No. We have five Physical Therapists on staff and one of us will be available to get you
going. You are also welcome to request a specific therapist to treat you.

Q Will you bill my insurance?
A Yes we will bill your insurance company directly. We are providers for most of the major     insurance companies (see list to right). Our office staff with verify your insurance benefits
and let you know if you have a copay or if your plan pays as a percentage. All patient's
do need to be aware that insurance companies sometimes process claims differently than
    our office was quoted.

Q What if I don't have insurance?
A Don't worry. Come on in. We do have self pay rates and if needed we can work out a
payment plan that fits your needs.

Q I am visiting from out of town. Will you bill my insurance?
A Again we are providers for several national plans, so if we are providers we will bill
your insurance. We do require a credit card for any remaining balance that might be
owed after you have returned home.
